Uniform Order Form 2017 - 2018
Notes:
* This Uniform Order Form is only available from April 20 - April 28, 2017.
* Please fill in all personal information completely and select the uniform that you want to order.
* If any uniform that you want to order are not listed , please contact the uniform committee.
* Please do the payment after receiving the confirmation (invoice) by email from the uniform order committees.
* Payment can only be done through transfer to SMAK PENABUR Kelapa Gading BCA account 2883206363 at the latest by  28 April 2017. Please write the invoice number and student's name on the transfer description.
* Please print the invoice twice and bring them together with the transfer slip to the school at the latest by 2 May 2017.
* The uniform committee will take one of the invoices and stamp the other invoice. The stamped invoice will be given to the students/parents for collection of the uniform on July 13 - July 14, 2017.
